Unleash Health & Safety Waiver Agreement

UNLEASH MOVEMENT — HEALTH & SAFETY WAIVER & PARTICIPATION AGREEMENT

Please read this carefully before attending any Unleash Movement classes or events.
By purchasing a credit, attending a class or event, and signing this waiver, you acknowledge and agree to the terms below.

 

1. Acknowledgement of Physical Activity

I understand that participating in movement classes or events — including but not limited to barre, yoga, strength, mobility, and stretching — involves physical activity that carries potential risks, including but not limited to:

• Muscle soreness, strains, or sprains

• Cardiovascular exertion or injury

• Accidental slips, falls, or other physical incidents

I confirm that I have voluntarily chosen to participate in this event/class with full knowledge of these risks.

 

2. Medical Clearance & Personal Responsibility

• I confirm that I am in good physical health and have no medical condition that would prevent me from safely participating in physical activity.

• If I am pregnant, postpartum, or have any medical conditions, injuries, or limitations, I take full responsibility to consult my GP or healthcare professional before attending.

• I agree to work within my own limits and modify exercises as needed.

 

3. Liability Release

I acknowledge that Unleash Movement and its instructors are not liable for any injury, illness, loss, or damage that may occur during or after the event, except in cases of proven negligence.

I accept full responsibility for my own physical and mental wellbeing while participating.

 

4. Safety & Conduct

• I agree to follow the instructor’s guidance and respect the shared space of all participants.

• I will alert the instructor immediately if I feel unwell, dizzy, or unable to continue.

• I will bring my own mat and equipment in good condition and will act responsibly during the session.

 

5. Photography & Media

• I understand that photos or short video clips may be taken during the event to capture the community experience.

• I consent to Unleash Movement using these for marketing purposes unless I have explicitly opted out in writing prior to the event.

 

6. PAR-Q Statement (Physical Activity Readiness)

By signing this agreement, I confirm that:

• I have answered “NO” to all standard PAR-Q questions regarding conditions that may make physical activity unsafe for me.

• If I have answered “YES” to any, I have obtained medical clearance.

• I understand it is my responsibility to update Unleash Movement if my health status changes.

 

7. Agreement

I have read, understood, and voluntarily accept the terms of this Health & Safety Waiver and Participation Agreement.

I understand that by attending any Unleash Movement event or class, I am fully responsible for my own safety and participation.
